Open up the EOS field on the Directory Listing form to allow CLEC requests for non-published service at no charge to flow through instead of being dropped for manual handling. Today, CLECs drop LSRs for manual handling and provide end user’s other service information in the remarks field when their end user customer qualifies for non-published listings at no charge. Opening this field would allow the CLECs to place this information on their DL form instead of in the remarks section and allow the LSR to flow through Qwest order creations systems.

| 11/14/07 Systems CMP Meeting Susan Lorence-Qwest stated that the following CRs would move to a CLEC Test status by the end of the week - Implementation Date 11/12/07. 10/17/07 Systems CMP Meeting Mark Coyne-Qwest reviewed the following change requests and associated implementation dates. SCR121306-01 Enable the EOS Field on the Directory Listing Form - Implementation Date - 11/12/07 1/17/07 Systems CMP Meeting Lynn Stecklein-Qwest stated that this CR is requesting to open up the EOS field on the Directory Listing form to allow CLEC requests for non-published service at no charge to flow through instead of being dropped for manual handling. She said that today, CLECs drop LSRs for manual handling and provide end user’s other service information in the remarks field when their end user customer qualifies for non-published listings at no charge. She said that opening this field would allow the CLECs to place this information on their DL form instead of in the remarks section and allow the LSR to flow through Qwest order creations systems. Dianne Friend-Comcast asked if this applied to Facility Based Listings. Lynn Stecklein-Qwest stated that she would find out. Bonnie Johnson-Eschelon asked it this CR includes only the Directory Listing Product, could they ask that this CR include both Facility Based Listings and Directory Listings. Lynn Stecklein-Qwest said that she would also see if that is possible. Status Update - 1/24/07 - Response to above questions - Because the billing system for FBDL has no way of being able to negate charges, the process does not include FBDL. It would entail re-architecture of back end systems.

| RESPONSE February 21, 2007 RE: SCR121306-01 Qwest has reviewed the information submitted as part of Change Request SCR121306-01. Based upon the scope of this CR as agreed to in the Clarification Meeting, Qwest is able to provide an estimated Level of Effort (LOE) of 1140 to 1900 hours for this IMA Change Request.
