| 11/18/09 Product/Process CMP Meeting Mark Coyne-Qwest said this change was effective on 10/31/09 and asked if there were any objections to closure. There were no objections. 9/16/09 Product/Process CMP Meeting Mark Coyne-Qwest said we will proceed with a level 2 to announce on 10/10/09 with an effective date of 10/31/09 8/19/09 ProductProcess CMP Meeting Cindy Schwartze-Qwest said we will be eliminating One Number Service, which is named Wireless Extension in all 14 state tariffs. The USOCs associated with this service are HME and HMP. The proposed implementation date is 10/31/09. Cindy said there are no wholesale customers and would like to reduce this from a level 4 to a level 2 notification. Julia Redman-Carter-PAETEC asked if this was being grandfathered and if there were any wholesale customers. Cindy Schwartze-Qwest said this is an elimination and there are no wholesale customers. Susan Lorence-Qwest said we will move forward with a level 2.
